R’ Yerucham Levovitz says that even though once a person transgresses they are liable to transgress again, if a person does immediate teshuva they do not fall into the pattern yet.

One of the most important things a person could do to improve themselves, is to do a dally reckoning of all their actions. This will help to take stock of themselves, improve areas that need improvement, and make Teshuva on their transgressions. While it might be unpleasant to remember transgressions, it is a great opportunity to erase them.
